Consumer Disclosure Regarding Conducting Business Electronically, Receiving Electronic Notices and Disclosures, and Signing Documents Electronically
Please read the following information.
This E-Sign and Consent Disclosure (“Disclosure”) explains your rights and responsibilities when you consent to receive notices, disclosures, agreements, statements, and other documents electronically (collectively, “E-Communications”) and utilize electronic signatures (“E-Signatures”) in lieu of paper documents executed with a handwritten signature. By consenting electronically, you confirm that you have reviewed this Disclosure and consent to conduct business with Park Bank using E-Communications and E-Signatures when permitted by law and bank procedure.
Scope of Your Consent
Your consent applies to E-Communications and E-Signatures related to any Park Bank product, service, application, account, loan, or other transaction (collectively, “Product”) you obtain, use, or maintain throughout your relationship with Park Bank, except where law or bank procedure requires a paper copy. If you open or use a new Park Bank Product in the future, this consent will automatically apply unless you explicitly withdraw consent for that Product as permitted by bank procedure. Withdrawing consent for one Product does not automatically withdraw consent for other Park Bank Products. E-Communications may be provided by email, through online banking, by notice that a record has been posted and instructions for accessing it, or by another electronic method permitted by law.
Withdrawing Consent and Possible Consequences
You may withdraw your consent to this Disclosure at any time by following the procedures below. Park Bank does not charge a fee to process a withdrawal of consent. However, withdrawal may affect your ability to use electronic services, online banking, receive electronic statements, or access products and services that are provided only or primarily through electronic channels.
Hardware and Software Technical Requirements
To access and retain E-Communications, you must have the following:
- A computing device with secure internet access.
- A current, standards-compliant web browser that supports HTTPS, HTML, and JavaScript, with standard encryption and cookie protocols enabled.
- A current version of software capable of viewing and downloading pdf files.
- Secure digital storage or printing capability.
- A current and active email address.
Consumer Purpose Products Only
Right to Receive Paper Records
You are not required to consent to this Disclosure. You may choose to receive E-Communications in paper form by closing this document and requesting paper copies. If you consent, you may still request a paper copy of any E-Communication or withdraw your consent at any time. Park Bank does not charge a fee to process a withdrawal of consent or to provide paper copies. You are responsible for promptly notifying Park Bank of any change to your email address or other electronic contact information using the procedures below.
Changes to Hardware and Software Technical Requirements
If Park Bank changes the hardware or software requirements in a way that creates a material risk that you will not be able to access or retain E-Communications, we will notify you. After receiving that notice, you may withdraw your consent without any fee and without any consequence that was not previously disclosed. Your continued use of E-Communications after receiving notice of the revised requirements will reaffirm your consent.

If contacting Park Bank by email, use the applicable subject line below and include the requested information so we can efficiently process your request:
“Requesting Paper Copies”: Include your name, telephone number, postal address, and the specific document or documents you want in paper form.
“Withdraw Consent”: Include your name, email address, telephone number, postal address, and the Park Bank product or account for which you are withdrawing consent.
“Update Contact Information”: Include your name, previous email address, new email address, telephone number, and postal address.
Consumer Affirmative Consent and Agreement
By consenting electronically, you confirm that you have reviewed this Disclosure and affirmatively consent to conduct business with Park Bank using E-Communications and E-Signatures when permitted by law and bank procedure, within the scope described above. You also confirm that you can access and retain this Disclosure and other E-Communications using the hardware and software outlined above. If you do not consent, close this Disclosure and contact Park Bank to request paper records or another available nonelectronic process.
Business/Commercial Purpose Products Only
By consenting to this E-Sign and Consent Disclosure electronically, you provide consent on behalf of yourself and the business entity you represent. (The “Business”)
Updating Representatives and Contact Information
The Business is solely responsible for:
- Maintaining current contact information for the primary corporate account and any individual Authorized Representative with Park Bank.
- Immediately notifying Park Bank of any necessary or desired changes to individual Authorized Representative’s rights and/or access to E-Communications.
Commercial Affirmative Consent and Agreement
By consenting electronically, you consent on behalf of the Business that:
1. You are an Authorized Representative with the legal capacity to bind the Business.
2. The Business meets the hardware and software technical requirements outlined above.
3. You have reviewed this Disclosure and affirmatively consent to conduct business with Park Bank using E-Communications and E-Signatures when permitted by law and bank procedure, within the scope described above.
